We recommend to first complete our new patient online questionnaire. Once submitted, you will have an option to provide payment via credit or debit card. Prior to your consultation, this information is required so that Dr. Krup may have a detailed background on your health and lifestyle to best maximize the value of your first meeting.
To begin, simply select our Female Questionnaire or Male Questionnaire. Information is securely processed and completion takes roughly 15 minutes. Once your information is received, our Patient Coordinator will promptly reach out for appointment scheduling.
